Every time I see a woman my age pregnant I get the empty feeling in my stomach. All I think about is babies. I shouldn't be having baby fever at 40. To compound things I have a 28 year old FWB. At 40, this is your body telling you there aren't many more chances. How is your life otherwise? Married? Previous kids? Money? You obviously think your FWB is good breeding stock. Barring other issues, I say ditch the IUD, stay off the pill, and let your lover fill your womb.
